Using the application to calculate risk involves entering four simple characteristics of patients with heart failure, including medication use, symptom status, exercise capacity, and a widely-used blood test in heart failure. Survival estimates are based on data analysis of 2255 patients with chronic heart failure managed at a specialized heart failure clinic, the Ahmanson-UCLA Cardiomyopathy Center. This application is for use by physicians who care for patients with chronic heart failure.
